JACKSONVILLE, Fla. (NNS) -- Preventive services are your best defense against preventable diseases. Your care teams at Naval Hospital Jacksonville (hospital and branch health clinics) stand ready to provide those services.

"Prevention is the best medicine," said Cmdr. Jennifer Wallinger, NH Jacksonville director for public health. "Annual women's health service exams are the best way to maintain your health and diagnose potentially serious conditions for successful treatment."

Many issues women face are preventable and treatable. The top two causes of death for women are heart disease and cancer, with diabetes ranking seventh, according to the Centers for Disease Control and Prevention.

Heart disease: Heart disease is the number one cause of death in U.S. women. Key risk factors include: high blood pressure, high LDL cholesterol, and smoking. Other risk factors include: diabetes, overweight and obesity, poor diet, physical inactivity, and excessive alcohol use. Know your blood pressure, talk to your primary care manager about diabetes testing, quit smoking, talk to your PCM about cholesterol and triglyceride tests, make healthy food choices, limit alcohol, and find healthy ways to cope with stress.

Cervical cancer: Prevent cervical cancer - with the right test at the right time. Get your first Pap test at age 21. If the test is normal, you can wait three years until the next Pap. After turning 30, you have a choice. Get a Pap test every three years. Or, get a combined Pap and human papillomavirus (HPV) test, and if both are normal, you can wait five years until the next screening. Talk with your PCM about options.

Breast cancer: Mammograms are the best way to find breast cancer early, when it's easier to treat and before it's big enough to feel. Starting at age 40, talk with your PCM about when to start and how often to get a mammogram. You have the choice to get an annual mammogram at age 40 and up. Call Radiology today (904-542-7782). It's easy to schedule, with no need to see your PCM.

Colorectal cancer: Colorectal cancer is the third most common cancer in U.S. women. If you're age 50 or over, get screened now for colorectal cancer. Screening can find growths so they can be removed before turning into cancer. Screening also finds cancer at an early stage, when treatment often leads to a cure. If you have risk factors, you might need to be tested earlier or more often. Talk to your PCM.

Diabetes: If you have diabetes, see your health care team every three to six months for an A1C test. This checks your long-term control of blood sugar. Make an appointment with your PCM today. You can discuss strategies to manage diabetes at home, work, school, and while traveling. You can also talk with a diabetes nurse or nutrition clinic, and ask about the Diabetes Wellness Team.

Chlamydia: Most people who have chlamydia don't know it, since there's often no symptoms. It's the most common sexually transmitted infection in the U.S. Women under age 25 (and older women with risk factors) need chlamydia testing every year. Call or email your PCM to schedule a urine sample in our lab.

Talk to your health care provider about non-physical concerns as well. Dealing with high stress, depression, difficulty sleeping, or other non-physical health concerns are just as important to stay healthy.

Proper sleep, physical activity, and nutrition are key components of preventive health and day-to-day wellness.

Women need a mix of cardio and resistance or weight-bearing exercise at least three to five times a week to help prevent osteoporosis, heart disease, cancer, and diabetes. Exercise also promotes good self-image, which is important to a woman's mental health.

Eating healthy is critical to women's health. A balanced diet of lean proteins, healthy fats, smart carbs, and fiber are essential elements to a proper diet.

For an annual well-woman exam, make an appointment with your PCM by phone (904-542-4677 at our hospital, or 904-546-7094 at BHC Jacksonville for active duty), or on TRICARE's Online Patient Portal at www.TRICAREonline.com.

For immunizations, stop by or call 904-542-7810 (hospital) or 904-546-7050 (BHC Jacksonville).

To take a free class on tobacco cessation, nutrition, or healthy weight, contact our hospital's Wellness Center at 904-542-5292.

For 24/7 clinical advice, call the Nurse Advice Line at 800-TRICARE (800-874-2273).

For secure email messaging with your care team (for non-urgent issues), sign up for free at https://mil.RelayHealth.com.

Paroles de la chanson Mama par Dalida
Oui je me revois
Une poupée dans les bras
Lui parlant tout bas
Me parlant de toi
Elle était si belle
Vêtue de dentelles
Elle me souriait
Et je te disais

Mama

Mama
Et nous serons très heureux ensemble

J'ai gâché mes jours
Sans trouver l'amour
Tu me l'avais dit
Quand je suis parti
Et quand le chagrin
Vous prend par la main
Alors on se souvient
Que sans toi on est rien

Mama
Quand ma poupée grandira
Je veux qu'elle te ressemble
Mama.

NASA's Mars rover Curiosity team is working
to restore Curiosity's sample-drilling
capability using new techniques. The latest development
is a preparatory test on Mars.

The five-year-old mission is still several months from the soonest possible
resumption of drilling into Martian rocks. Managers are enthusiastic
about successful Earth-based tests of techniques to work around
a mechanical problem that appeared late last year and suspended use
of the rover's drill.

"We're steadily proceeding with due caution to develop and test ways
of using the rover differently from ever before, and Curiosity is
continuing productive investigations that don't require drilling,"
said Deputy Project Manager Steve Lee, of NASA's Jet Propulsion Laboratory,
Pasadena, California.

Curiosity touched its drill to the ground Oct. 17 for the first time in 10 months.
It pressed the drill bit downward, and then applied smaller sideways forces
while taking measurements with a force sensor.

"This is the first time we've ever placed the drill bit directly
on a Martian rock without stabilizers," said JPL's Douglas Klein,
chief engineer for the mission's return-to-drilling development.
"The test is to gain better understanding of how the force/torque sensor
on the arm provides information about side forces."

This sensor gives the arm a sense of touch about how hard
it is pressing down or sideways. Avoiding too much side force
in drilling into a rock and extracting the bit from the rock is crucial
to avoid having the bit get stuck in the rock.

Curiosity has used its drill to acquire sample material
from Martian rocks 15 times so far, from 2013 to 2016.
It collected powdered rock samples that were delivered
to laboratory instruments inside the rover. On each of those occasions,
two contact posts -- the stabilizers on either side of the bit --
were placed on the target rock while the bit was in a withdrawn position.
Then a motorized feed mechanism within the drill extended the bit forward,
and the bit's rotation and percussion actions penetrated the rock.

The drill's feed mechanism stopped working reliably in December 2016.
After exploring possibilities of restoring the feed mechanism's reliability
or using it despite unreliability, the project set a priority to develop
an alternative method of drilling without use of the feed mechanism.
The promising alternative uses motion of the robotic arm to directly advance
the extended bit into a rock.

"We're replacing the one-axis motion of the feed mechanism with
an arm that has five degrees of freedom of motion," Klein said.
"That's not simple. It's fortunate the arm has the force/torque sensor."

The sensor's main use until now has been to monitor for a force
so excessive of expectations that it would automatically halt all arm motion
for the day. The new "feed-extended" drilling uses it to compensate
for side loads. This test will help engineers determine how data from
the sensor can be used most effectively.

Using this method, a near-twin of Curiosity at JPL has collected drilled
samples from Earth rocks. The team has also developed methods
to deliver drilled samples to the laboratory-instrument inlets
on the test rover's deck without use of the drill's feed mechanism.
Development of this alternative sample-transfer technique is needed
because the process used previously depended on having the bit
in a withdrawn, rather than extended, position.

"The development work and testing here at JPL has been promising,
" Lee said. "The next step is to assess the force/torque sensor on Mars.
We've made tremendous progress in developing feed-extended drilling,
using the rover's versatile capabilities beyond the original design concepts.
While there are still uncertainties that may complicate attempts
to drill on Mars again, we are optimistic."

The rover's current location is on "Vera Rubin Ridge" on lower Mount Sharp.
Curiosity is nearing the top of the 20-story-tall ridge.
It has been studying the extent and distribution
of the iron-oxide mineral hematite in the rocks that make up
the erosion-resistant ridge.

During the first year after Curiosity's landing near Mount Sharp,
the mission accomplished a major goal by determining that, billions
of years ago, a Martian lake offered conditions that would have been
favorable for microbial life. Curiosity has since traversed through
a diversity of environments where both water and wind have left their imprint.
Vera Rubin Ridge and layers above it that contain clay
and sulfate minerals provide tempting opportunities to learn even
more about the history and habitability of ancient Mars.

Annual mammograms recommended for women over 40

Medical officials recommend women age 40 and older have a mammogram
annually to detect for signs of breast cancer.

“Mammography has helped reduce breast cancer mortality
in the U.S. by nearly 40 percent since 1990. Annual mammograms
can help detect cancer in its earliest stages when it’s most treatable,”
said Maj. Sara Michael, a diagnostic radiologist and Chief of Mammography
and Ultrasound at Blanchfield Army Community Hospital.

A mammogram is an x-ray that shows the tissue inside of a breast.
The x-ray creates an image that a radiologist like Dr. Michael can use
to look for abnormalities in the breast. In most cases a patient is fine
and won’t need another mammogram until the following year.
Regular mammograms can better help your healthcare team
in detecting cancer by giving them a progressive look
at your breast over time.

“For every 1,000 women who have a mammogram,
about 100 are called back for another look with an additional mammogram
or an ultrasound. If the Radiologist sees an abnormality,
then a biopsy can be ordered which allows the medical team
to collect tissue from the breast for a pathologist to evaluate
and determine if cancer is present,” said Michael.

A mammogram can detect signs of cancer well before it can be detected
during a self-breast exam, which is the reason women over 40 are encouraged
to have the procedure annually. Detecting cancer early improves
a woman’s chance of a successful treatment.

Aside from some skin cancers, breast cancer is among the most common cancer
affecting women. More than 200,000 cases are diagnosed annually.

Women over age 40 who receive care from BACH and have no current health
issues can call directly to schedule their mammogram. Patients under medical care
at BACH for a current health issue should speak with their physician or provider
for a referral. Women over age 40 enrolled in TRICARE, who receive
their medical care off post can ask their physician or provider
for a referral to receive a mammogram at BACH. According to the TRICARE website,
women beneficiaries over 40 are authorized a mammogram annually. Additionally,
younger patients with certain risk factors can ask their healthcare physician
or provider for a referral and use risk models to determine if additional screening
measures are needed.

Dr. Michael recommends not having a mammogram the week before
or during your period. Breasts may be tender or swollen during that time causing
discomfort during the procedure. Once a mammogram is scheduled, Dr. Michael offered
the following tips:

Don’t apply deodorant, perfume, or powder before your mammogram because
these can show up as white spots on the X-ray.

Dress comfortably in a top and bottom so you can easily undress
from your waist up for the mammogram.

Plan to spend about 30 minutes in the clinic. While the mammogram takes
just minutes, patients should allow time to complete paper work.

Once completed, patients will receive a letter in the mail explaining in easy
to understand wording what, if any, next steps need to be taken.
The clinic personnel will call patients who need additional imaging due
to an abnormality seen on their screening mammogram, so it is very important
to ensure a proper phone number is on the intake form.

Finally, mammograms work best when they can be compared with previous ones.
This allows the radiologist to compare them to look for changes in your breasts.
If you have had a mammogram at another facility, you will need to fill out the necessary
forms to have those images mailed to BACH. Patients who have had a mammogram
at BACH and are transferring to another installation can visit the clinic to get a copy
of their mammogram images to give to their new provider.

“Patients can fill out some paperwork and request a CD before leaving
the department and the CD will typically be ready in 48 hours. While another medical
treatment facility should be able to transmit the images electronically,
I still recommend having a CD on hand since plan B is always advised,”
said Dr. Michael, adding that BACH will also accept mammogram images
for incoming patients.
